5 Best Practices You Need to Know

Root Cause Analysis Best Practices

What Are These 5 Best Practices?

Here are five best practices that you need to understand and implement:

  1. How to use peer reviews and inspections to achieve high reliability.
  2. How to manage fatigue in your workforce.
  3. What a good procedure looks like.
  4. What is psychological safety and how can you foster it in your organization?
  5. Why do you need to train people beyond potential mistakes?

That’s a great list, but where can you find out more? Read on…

5 Best Practices Sessions

The 2025 Global TapRooT® Summit has eight Best Practice Tracks. Each track has nine best practice sharing sessions. The list above comes from five different sessions in three different tracks. Below we will discuss each of the sessions.

1. How To Use Peer Reviews and Inspections To Achieve High Reliability

This session is in the High-Reliability Organization Best Practices Track organized by Mark Paradies.

This session is a panel discussion where Mark Paradies, Gard Clark, Ken Reed, and Justin Clark will share their experience with different types of peer reviews and inspections used in the Nuclear Navy to achieve high reliability. Then the session will open up for questions and suggestions from attendees.

If you’ve ever wondered about some of the techniques used by Admiral Rickover and the Nuclear Navy to achieve high reliability, this is your chance to hear from people who have “been there – done that” and have the scars to prove it. Plus, you can ask questions for an even deeper understanding.

2. How To Manage Fatigue in Your Workforce

This session is in three tracks, including the High-Reliability Organization Best Practices Track, the Safety & Risk Management Track, and the Human Performance Best Practices Track.

This is a presentation by Bill Davis of Cercadian Technologies about fatigue and human error and how a company can manage fatigue to reduce human errors. This session is especially important for people who run 24/7 operations in high-risk industries (nuclear, utilities, hospitals, refineries, oil platforms, chemical plants, …).

3. What a Good Procedure Looks Like

Procedure writing and use for high-reliability

This session is in two tracks, including the Human Performance Best Practices Track, and the High-Reliability Organization Best Practices Track.

If you believe that clear, usable procedures are important, then this session is for you. Any Souders’ interactive presentation will focus on procedure writing best practices from high-reliability organizations. This session is fundamental to anyone looking to improve human performance.

4. What is Psychological Safety and How Can You Foster it in Your Organization?

This session is in two tracks, including The Psychology of Improvement Best Practices Track, and the Human Performance Best Practices Track.

Barb Carr of System Improvements will guide attendees on an exploration of the concept of psychological safety and its impact on employee well-being, engagement, and performance. The session will provide practical strategies for fostering an environment where employees feel comfortable expressing ideas, taking risks, and learning from mistakes without fear of negative consequences. Attendees will learn how to identify and address barriers to psychological safety, create open lines of communication, and cultivate a culture of trust and respect. Whether you’re a leader, manager, or team member, this session equips you with the tools to transform workplace stress into a pathway for success, enhancing collaboration and driving positive outcomes.

5. Why Do You Need to Train People Beyond Potential Mistakes?

This session is in four tracks, including Human Performance Best Practices Track, the Psychology of Improvement Best Practices Track, the High-Reliability Organization Best Practices Track, and the Instructor Recertification Track.

Erika Armstrong, pilot and VP at Advanced Aircrew Academy, will present a concept used to make aviation a high-reliability organization. Pilots, operators, or mechanics are trained to do things the “right” way. But what happens when they make a mistake? That’s where “training beyond the mistake” is needed. Erika will share her ideas and answer your questions about this topic.

What Else is at The Summit?

First, these are just five of the 40+ best practice sessions at the 2025 Global TapRooT® Summit. Some of these sessions are about the TapRooT® System and Software for root cause analysis.
