Fatigue Causes Incidents/Accidents

Is Fatigue a Root Cause?

I believe that fatigue is one of the most underreported root causes of incidents and accidents.

Why do I believe that? The answer is in the video below where I discussed the 7 Deadly Sins of Human Performance at the 2016 Global TapRooT® Summit.

If you missed the Summit in 2016 or maybe you’ve forgotten the presentation (certainly there has been a lot happen since 2016), maybe now is a good time to watch this short (about 3 minute) video to learn more.

Fighting Fatigue Session at the 2021 Global TapRooT® Summit

Plan ahead and learn more about fatigue at the 2021 Global TapRooT® Summit. The Stopping Human Error Track has a session just about fighting fatigue.

Dr. Erin Flynn-Evans

Dr. Erin Flynn-Evans, NASA Fatigue Researcher, will discuss fatigue research and countermeasures NASA has developed to fight fatigue.

Then Darrell Graham from Aware360 will present a technology solution for measuring fatigue: Aware4Duty

This is just one of the great sessions in the Stopping Human Error Track at the 2021 Global TapRooT® Summit. See:
