“Reap the Rewards From Great Root Cause Analysis” is a tremendous kickoff session in the Investigations and RCA Best Practices Track at the 2025 TapRooT® Summit.
We have a couple of guest speakers who will share their own stories to help us all learn how to get great results from our investigations and RCA best practices, including how to use the RCA results to drive meaningful change.
Our first speaker is Richard Hoffman.
Richard has provided EHS leadership in many roles. He has served as the Vice President of Health, Safety, Environmental, and Quality (HSEQ) for U.S. Well Services and as VoltaGrid’s Director of HSEQ.
He is now the EHS Director for Washington Health Systems. He has over 25 years of experience in the HSE field. Richard has received extensive training in root cause analysis programs and has a proven track record in successfully implementing safety culture as a fundamental operating tenet.
Richard is a member of The American Society of Safety Professionals, The National Safety Council, National STEPS, and other professional organizations. He also serves on the User’s Advisory Board for TapRooT®.
During the session, we will learn how Richard and his team significantly reduced loss time incidents and kept clear of OSHA Recordables by leveraging TapRooT® RCA. Goals most of us would like to achieve!
After that talk, Cory Wald will share his experience of avoiding a heavy fine by using TapRooT® RCA. Cory is an experienced SHE&S Manager who knows that a strong safety culture is built on the strength of an organization’s people and their commitment to the values.
After nearly 10 years of helping create one of the best-performing organizations in the transportation industry, Cory brought his skillset and passion for safety to Nutrien. 17 years later, Cory is proud to be part of a top-performing organization that fosters innovation and ownership among all levels of employees.
