Are These Root Causes?

Headline: “Inattention top cause of workers being struck by mobile equipment” … Are These Root Causes?

I saw an article from a safety organization with the headline above. The article went on to say that the “top 10” root causes for being struck by mobile equipment for people working in the corrugated industry are:

  1. Operators, pedestrians, and others in the workplace not paying attention to their surroundings
  2. Lack of traffic management plans
  3. Lack of training
  4. Lack of enforcement of policies
  5. No commitment to put measures in place to reduce risk
  6. Lack of safety processes
  7. Poor work environment (lack of rules/lack of enforcement with high-visibility vests)
  8. Lack of, or ineffective, policies
  9. Improper use of mobile equipment
  10. Not learning from incident reports (lack of awareness to take these incidents more seriously)

What do you think? Were these results from a facilitated root cause analysis with experts really root causes?
