Three workers were injured in a building collapse incident. One had to be extricated and almost lost a leg. Less than a day later, at another work site, a worker fell 30 feet.

How could another building collapse incident be avoided?

So much thought needs to go into corrective actions for high-consequence incidents. There are certain corrective actions that everyone seems to gravitate to but they are often ineffective.
