Procedure Writing and Use for High Reliability
Have you ever noticed how many major accidents have at least one cause related to procedures either not being used or being used improperly? High-reliability organizations (nuclear plants, commercial aviation, and nuclear weapons maintenance are three examples) have accurate, well-written procedures and insist that people in the field use them.
