On the North Slope, we take pride in our environmental and safety record. Any significant environmental or safety incident is carefully investigated and fixed. The trick is to focus on fixable causes – not blame.

In this incident, a truck driver spilled his load of 30 tons of steel next to and under a natural gas liquid pipeline. No environmental damage was done and no one was injured, but it could have been worse.

The contractor driver was the only witness to the accident and we needed his cooperation. We explained that we would have him tell us his story and we would draw a SnapCharT® to lay out the sequence of events. He seemed comfortable and began.

What followed was amazing to us (two experienced investigators). The more he talked, the more we realized how little we knew. Two major Causal Factors emerged. 1) The distracted driver was using his cell phone (called his boss because he was late) and jerked the wheel while in a turn. 2) His load shifted causing the roll over.

The investigation found several important root causes that we corrected. First, at the time there was no regulation of cell phone usage for truck drivers. That changed (before any state or federal laws were proposed). Second, we found that training was needed about securing loads for the non-DOT roads used on the North Slope (even for experienced drivers).

You might think the driver would be fired for this type of accident. However, due to the investigation, his employer recognized that this accident could have happened to anyone. The driver was a valuable asset. He is now helping raise awareness about the hazards of cell phone use even on low speed, non-DOT roads.
